This is very very scary indeed--and one wonders about why we're seeing reporting of hacking all the time--we have to do better--no question.

Security Lags Behind
I worked at one company and they had not patched the employee computers in over six years and you would be surprised how often this happens at major companies including IT companies.
I agree that security must be included in the design and must continually be monitored, updated and upgraded because security threats, viruses and hackers are here to stay.

I am not surprised. Companies only become concerned about security when customers do or until there is a data breach or major security threat.
Companies fail to realize that is costs exponentially more money to fix a security issue by being reactive than to be proactive and implement security measures to reduce security risks.
